To the Honorable Adjutant General Air National Guard. My name is Jorge Estarellas and I am requesting copy of my 214 Form. Prior to this communication Mr. Richard Rudnick was contacted by e-mail (rich@nvf.org) and he said that the copy which I need can be request to you, due to the fact that I served here in Puerto Rico.
